If you have been assigned to assist a new applicant in the composition of a libellus to formally begin a nullity trial, please complete both of the following forms. We cannot begin the trial without both documents.

What is required for a valid libellus?
The Instruction Dignitas connubii, article 116, states that the following are required for a valid libellus:
You will be notified when it is time for the publication of the acts. At this time, you and the parties have the right to review the procedural decrees, testimonies, and any other proofs introduced in the case.
Please contact the party whom you represent and find out if he/she wants to examine the acts and read the testimony. If not, you still have the right to view the acts. It is important that you do so.
If for some reason neither you nor the party desire to view the acts, please submit an Acknowledgement of Notification form.
If a party requests that additional testimony or proofs be obtained or if the judge determines that additional testimony or proofs are needed, there will be a second Publication of the Acts so that the parties and their advocates can examine the new information.
If an affirmative decision was rendered and neither the parties nor the defender of the bond have challenged the sentence within the necessary amount of time, then the sentence will be executed and the parties will receive a decree stating that they are free to marry in the Catholic Church.
